Requests for notification of purpose of use, disclosure of personal data, etc. can only be made by the person in question or his/her representative.
To confirm that the requestor is the person in question or a representative, please first contact our dedicated Personal Information Contact Us Desk (see below) with your request for disclosure of personal information. We will send you a "Personal Information Contact Us Application Form" as prescribed by us.
Please fill in the required information, enclose the identification documents listed on the form and 300 yen in stamps for the actual cost of the work (only if you wish to receive notification of the purpose of use and disclosure of personal information), and send it again by mail to our Contact Desk. (If you are using an official certificate as identification, please fill in the black text except for the part of the certificate that identifies you, such as name and address.) If the "Personal Information Contact Us Application Form" is incomplete or if the identity of the applicant cannot be confirmed, we will contact the applicant to that effect. Please note that it may take a few days for the form to arrive in the mail, although we will respond as soon as possible after receiving it.
